This quest is called 'Cheating Death', for Adventurers levels 240 to 241.
Description of Cheating Death:
As you know, I miss my husband greatly since his untimely departure from
the mortal plane. I have done nearly everything to revive him and to
transform him back from the monstrosity he has been transformed into, but
everything thus far has failed. Your objective in this quest is to visit
Death to see if he can intervene. First, gather the implements necessary to
do a reading on my Lord, and then travel to the void and see if you can find
anything out from Death himself. Hopefully, going to such extremes will
convince the Powers of my resolve, and convince them to bring Lord Templeton
back to me. Your service will be well rewarded if you help me get my
beloved Lord back! May the Powers guide your steps!
